Position Title: Nurse Practitioner

Department: PACE

Supervisor: PACE Medical Director

Status/Hours per week: Exempt / Full-time

Primary Function: Responsible for primary care of PACE (Program of All-inclusive Care for the Elderly) participants.

Duties & Responsibilities:

* Act as a primary care provider for PACE participants in community and nursing home settings.* Provide routine, episodic, and urgent care.* Establish collaborative practice with PACE primary care physician.* In conjunction with primary care physician, perform semiannual assessments and examinations of PACE participants.* Together with the interdisciplinary team, develop care plans for PACE participants.* Work collaboratively with members of interdisciplinary team.* Communicate with consultants and integrate recommendations into plans of care.* Prescribe in accordance with state regulations and Upham's Corner Health Center policy.* Provide education and support to PACE nursing staff.* Attend interdisciplinary team meetings.* Maintain appropriate medical records and related documentation.* Maintain appropriate licensure.* Practice in accordance with appropriate PACE guidelines.

Qualifications

Minimum Basic Knowledge:

* Experience working in an interdisciplinary team.* Comfort working in a multicultural health care environment.* Knowledge and comfort working in a managed care setting.

Experience & Qualifications:

* Advanced practice nurse with MSN degree and board certification.* Licensed as Nurse Practitioner in Massachusetts.* Interest and minimum of one year of experience working with geriatric population.
