Position Summary:
The Codman Square Health Center is seeking primary care Nurse Practitioners and Physician Assistants to work in the Health Center's Family Medicine and Internal Medicine Departments. The Health Center serves a diverse community and offers an array of services including primary care, urgent care, behavioral health, care management, nutrition, family planning, optometry, dentistry, laboratory, radiology, pharmacy, and medical specialty services, including HIV care, Substance Use care, Hepatitis C treatment, OB-GYN, Nephrology, Cardiology, and an STI clinic. The primary care departments provide highly collaborative team atmospheres, with integrated behavioral health, care management, social service, nutrition, and family planning services to collaborate with and support the primary care providers. In addition to performing primary care, opportunities are available for primary care providers to lead medical group visits and to provide medical subspecialty services, including HIV, substance use, and Hepatitis C care, as well as, working in our urgent care and STI clinics. The Health Center is a teaching institution and opportunities for NP resident and NP/PA student teaching are available. The Health Center has a close partnership with Boston Medical Center and is a member of the Boston Accountable Care Organization.

Primary Care Providers are integral in achieving this vision by providing high quality coordinated team based clinical care, being leaders on care teams, and sustaining the health center through billable visits. The PCP is responsible for managing their panel of patients through direct patient care and supporting population health management with an integrated care team. The PCP is a participant and leader in health center improvement work, and supports practice transformation including transformation into a Patient Centered Medical Home.
In addition, the Health Center is seeking Anchor Nurse Practitioners (Anchor NP's) who are a member and extender of a primary care team that includes primary care providers, nurses, medical assistants, and other integrated team members. The Anchor NP provides direct patient care, supports timely completion of care for the team, and co-manages selected patients with the primary care providers and other team members, including the complex care management team, the integrated behavioral health team, and the clinical pharmacy team.
The Anchor NP enhances timely access for health center patients and improves the patient experience for patients on the panels of the primary care providers in their department by:
* Providing direct patient care as needed or as requested to patients on the panels of the primary care providers on the team;
* Providing enhanced/more frequent care to high risk patients and patients with chronic diseases;
* Providing specialized care responsibilities for patients with certain conditions at the discretion of the Medical Director; and
* Providing inter-visit support for the primary care team and coverage for unavailable providers.
Qualifications:
* Current Massachusetts Nurse Practitioner Physician Assistant License * Current DEA and Massachusetts Control Substance License * Experience using Epic EHR preferred * Haitian Creole, French or Spanish language skills preferred
