Overview
Supporting Community Healthcare is a rewarding role. LifeLong Medical Care is looking for a Community Health Worker (CHW) - Prenatal to work at our William Jenkins Health Center in Richmond, CA. The Prenatal CHW's responsibilities include welcoming our patients, making basic psychosocial and nutritional assessments, offering appropriate information, education and referrals, and providing emotional support to a culturally and socioeconomically diverse population. The CHW will work closely with the medical providers and medical assistants. The ideal candidate is someone who likes to work with a team, is familiar with local community resources, and has experience working with women or in a community health setting.
This is a full time, 40 hrs/wk, benefit eligible position.
This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Responsibilities
Patients will receive care that addresses psychosocial, health education, and nutritional needs. Care will be in compliance with the guidelines of the Comprehensive Perinatal Services Program (CPSP).
* Provide orientation and complete assessments on pregnant patients* Being a good listener and non-judgmental to capture accurate information for the chart* Management of active pregnancy panel with accurate data for the prenatal department* Case management of pregnant patients needing extra support to navigate referrals and other supportive programs during prenatal/postpartum/post-miscarriage care* Open new pregnancy episode in EPIC system, tracking missed appointments, and closing pregnancy episodes* Create plan and provide appropriate interventions following steps to care guidelines* Provide health education, support, referrals, and follow-up to patients on caseload* Seek donations of car seats or other items for individual patients as appropriate* Identify high risk patients and refer to psychosocial services* Meet all mandated reporting requirements and report to medical providers within 24 hours as needed* Medical/Insurance knowledge to assist in navigating the health system as needed* Document all interactions with patients within 24-48 hours for billing/internal communication purposes* Complete all paperwork required for CPSP to stay compliant* Attend and participate in meetings* Coordinates with Lifelong medical staff, and other organizations to resolve patient issues* Consult with providers and/or psychosocial services to ensure appropriate services* Perform other duties as assigned by supervisor

* Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins* Strong organizational, administrative, multi-tasking, prioritization and problem-solving skills* Excellent interpersonal, verbal, and written skills* Conduct oneself in internal and external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff* Work in a team-oriented environment with a number of professionals with different work styles and support needs* Ability to work effectively under pressure in a positive friendly manner and to be flexible and adaptive to change* Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy* Maintain strict confidentiality with patient care* Ability to effectively present information to others, including other employees and community partners* Dependable in attendance and good time management to complete tasks* Be creative and mature with a "can do", proactive attitude and an ability to continuously "scan" the environment, identifying and taking advantage of opportunities for improvement
Job Requirements
* High school diploma or GED* One-year work experience with prenatal/obstetrics patients* Knowledge of lactation education and/or resources* Experience working in a non-profit clinic or hospital setting* Experience with electronic health records* Knowledge of or the ability to learn community resources* Ability to interact effectively with community agencies and to advocate for patients needing services* Comfort with discussing sensitive topics such as sexuality, interpersonal violence, financial issues, and substance abuse* Proficient in Microsoft office Word, Excel, Outlook* Must be bilingual English/Spanish.
