SCHEDULE: Full Time; 32 hours per week
LOCATION: Davis, CA
PAY RANGE: $27.19 to $33.23 hourly

Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Perinatal Health Educator works as a member of a multidisciplinary team which includes social workers, dietitians, medical assistants, medical practitioners including midwives and physicians, and other clinic staff to provide comprehensive care in a non-judgmental manner. Health education is part of a comprehensive approach to reducing the incidence of low birth weight babies. The Clinics emphasize prevention and health education in all services. In addition, the PN Health Educator provides administrative support
EDUCATION, EXPERIENCE, TRAINING
* High school diploma or GED required* College degree preferred* Experience in a medical office, clinic or hospital preferred* Experience in prenatal and postpartum care or Women's health services is preferred.* Bilingual English/Spanish preferred
KNOWLEDGE OF WORK

Effective communication skills and ability to encourage patient understanding or prevention practices
2.
Expected to have strong interpersonal skills & have the ability to understand and follow written/verbal instructions.
3.
Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.
DUTIES AND RESPONSIBILITIES
1.
Provide one-on-one Comprehensive Perinatal Services Program (CPSP) services including: initial patient orientation, financial screening, assessment of psychosocial, nutrition and health education needs and care plan development based on needs.
2.
Perform reassessments at each trimester and at two weeks postpartum with care plan updates as needed.
3.
Provide perinatal health education throughout patients' pregnancy.
4.
Consult and refer per CPSP guidelines to Social Worker, Nutritionist, Teen clinic, Dental Clinic, etc.
5.
Complete chart documentation as required by CPSP.
6.
Review charts prior to clinic to identify clients in need of CPSP services and/or health education.
7.
Track patient case load to ensure patients are scheduled and attending reassessment visits, education visits, classes and referral appointments.
8.
Provide education on a variety of pregnancy and postpartum topics per CPSP guidelines.
9.
Teach prenatal classes on a variety of prenatal and postpartum topics including but not limited to: prenatal nutrition, breastfeeding, labor and birth and infant care
10.
Under the direction of a midwife, lead diabetic group (Sweet Success) and prenatal group education.
11.
Attend monthly staff meetings and trainings.
12.
Other duties as assigned.
