Responsibilities
* Conducts enrollment assessments to determine clinical eligibility.* Completes UAS - NY Assessment instrument.* Maintains frequent communication with the Home Care Department and Care Management to keep ArchCare Senior Life a primary option for care delivery.* Identifies any barriers to implementation of Plan of Care.* Communicates with the Care Team in conveying information to facilitate any concerns, or answer questions.* Completes UAS - New York Assessment instrument as assigned to determine continuing eligibility and / or identify any needed changes to Care Plan.* Performs other related duties, as required.
Job Type: Per Diem
Salary: Fee-for-service structure
$225 per assessment, $250 per new enrollment
Training is compensated at $40.00 per hour
Qualifications
* Registered Nurse, currently licensed and registered to practice in New York State.* UAS-NY Certified* Bachelor's Degree in Nursing preferred.* Experience in Home Care, case management, or long-term home care.* Demonstrated skills in planning, organizing and directing the work of others required.* Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.* Proficient with DOH UAS assessment instrument.
