FlexStaff Our client is seeking an experienced and motivated Clinical Assessment RN Manager to lead our clinical assessment team and ensure the efficient and timely completion of all tasks associated with the clinical assessment process. The ideal candidate will play a key role in managing the onboarding and reassessment of participants, ensuring accurate completion of the Uniform Assessment System of New York (UAS-NY), and collaborating with care delivery teams. This position involves conducting assessments, medication reviews, and post-hospital discharge evaluations while complying with all Department of Health (DOH) policies and regulations. Key Responsibilities: Conduct in-home or virtual clinical assessments using UAS-NY for prospective and enrolled PACE participants. Review and update previous assessments, primary diagnoses, and medications in electronic medical records (EMR). Collaborate with Intake, Enrollment, and Care Delivery teams to ensure smooth transitions and optimal care for participants. Develop individualized service plans, provide education on the PACE program, and promote participation. Ensure timely completion of documentation within 48 hours and maintain accurate clinical records. Engage in continuous quality improvement and provide feedback to enhance processes and participant outcomes. Serve as the clinical point of contact for Care Delivery teams and provide ongoing support after participant enrollment. Comply with all CMS/PACE guidelines and organizational policies. Qualifications: Education: Bachelor of Science in Nursing (BSN) preferred. Experience: 3-5 years in Managed Care Programs (MLTC, LTHHCP, MAP, PACE, etc.). 5+ years in case management, disease management, utilization review, or quality assurance. Experience working with frail or elderly populations. Proficiency in UAS-NY and Lenavi. License: Active and unrestricted RN license in New York State. Strong clinical management and customer service skills. Bilingual skills are strongly preferred.
