Summary
Under the direction of the Department Supervisor, this position performs assigned functions for continuity of patient care and efficient patient flow. Need to maintain a high quality of patient care.
The Certified Nurse-Midwife provides primary women's health care focusing on pregnancy, childbirth, and the postpartum period, care of the newborn and the family planning and gynecological needs of women. The nurse-midwife exercises a high level of independent judgment in managing patient care assessing risk status.
The Certified Nurse-Midwife practices within a health care system that provides for physician consultation, collaborative management and referral as indicated by the health status of the patient. The nurse-midwife is responsible for providing full-scope care, which includes twenty-four-hour in-house coverage and outpatient care six days a week. The nurse-midwife is administratively accountable to the Chief Operating Officer.
ESSENTIAL DUTIES
1. Demonstrates obstetrical and gynecological theoretical knowledge, hand skills; follow clinical guidelines, policies and procedures.
2. Provides primary care in an ambulatory setting to women including timely ordering and accurate evaluation of diagnostic tests and management of minor complications. Identifies factors that indicate deviations from normal and provides follow-up services according to the standards delineated in the Nurse-Midwifery Scope of Practice.
3. Performs a comprehensive history and physical exam in an accurate and timely manner. Develops and implements a plan of care with the patient which is consistent with the clinical objectives of the multi-disciplinary obstetrical team.
4. Prescribes appropriate treatments and medications from an approved formulary for the management of a client's contraceptive needs and/or for specific medical problems.
5. Identifies deviations from normal and consults, co-manages or refers as appropriate.
6. Provides limited obstetric ultrasound as indicated and according to established clinical guidelines.
7. Provides individualized client teaching and counseling utilizing the nurse-midwifery philosophy, as appropriate.
8. Recording of Patient's Progress, Procedures, and Other Matters:
a. A visit will be recorded, and the chart note accepted in the EHR within seven days of the encounter.
b. Notes should:
i. Reflect change in a patient's condition and other significant information.
ii. Include documentation of pertinent history, review of systems, and physical exam
iii. Indicate studies performed and that results are reviewed and discussed with the patient.
iv. Reflect provider's thought processes as to the patient's problems, diagnosis and treatment
v. Document the need for continuing services and the appropriateness of diagnostic evaluations and treatment plans.
vi. Include referrals to specialty clinic services as determined to be medically necessary.
vii. Specify medications prescribed by the provider
viii. Reflect any pertinent attitudes or behavior on the part of the patient or family towards any aspect of care.
ix. Include notation of the need for routine and preventive services such as screening tests, periodic laboratory examinations related to chronic disease care, and immunizations
c. Results (including but not limited to labs, radiology results, consult notes, outside records) received by the provider should be attended to within 7 days, with acutely abnormal results attended to more urgently.
d. Use, protects, and discloses UMMA patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
9. Ensure compliance with productivity and documentation standards including:
a. Meeting goal of 16-19 patients to be seen in an average 8-hour day.
b. Complete all documentation, coding and data collection meet schedule and accuracy requirements for UDS data collection purposes.
10. Participates in a continuous, collaborative peer review process and in audits to conform to regulatory standards.
11. Performs other duties as assigned.

Individual abilities may result in some deviation from these guidelines.• To perform effectively in this position, the incumbent must have:• Experience: Clinical experience (full-time) within the last 3 years.• Education: Master's degree preferred and Graduate of an accredited school/university of nurse-midwifery.• Specialized Training: Current certification in CPR, Current certification in Neonatal Resuscitation is recommended, Credentialed at CHMC as an Allied Health member of the Department of OB/GYN, Certification in Limited OB ultrasound.• Licensure: Current licensure in the state of California (RN and CNM), certification by the American College of Nurse-Midwives (ACNM) or the ACNM Certification Council, current furnishing number and DEA license.
