Description:
Assesses and identifies the vascular access needs of patients with late stage Chronic Kidney Disease (CKD), and End Stage Renal Disease (ESRD). Provides professional case management for all Chronic Kidney Disease (CKD) and End Stage Renal Dialysis (ESRD) patients presenting for consultation or followed by the Vascular Surgery Service. Procures, delivers, and coordinates services for patients in the outpatient dialysis facility, outpatient clinic, and hospital setting. Ensures ongoing monitoring of patient's vascular access to assure adequate dialysis is obtained, maximum life of the access is achieved, and plans for future intervention are coordinated if necessary.
Reports all regional information related to the vascular access program to Program Office as requested. Provides education to kidney disease patients through any necessary means, primarily to include face-to-face office visits, face-to-face hospital visits, education classes in the medical office buildings, and in the outpatient dialysis units, and less commonly telephonic counseling. Works diligently to contribute to the improvement of the Optimal Starts metric for the region.
Roles: Processes all CKD and ESRD patients presenting with vascular access complications and assumes triage responsibilities for CKD and ESRD patients. Acts as liaison between the Department of Nephrology, contracted dialysis units, contracted vascular access centers, etc. to facilitate the healthcare needs of the defined patient population (e.g., ESRD patients) and to assure the appropriate level of continuity of care. Expedites surgical procedures related to vascular access issues as appropriate. Works with other members of the healthcare team to facilitate clinic appointments in order to assure compliance with specific departmental vascular access goals. Works with outpatient dialysis units in the identification of patients with potential vascular access problems in order to facilitate early detection as well as timely intervention. Works closely with patients to assure that they comply with prescribed plans of care and adhere to established appointment schedules, i.e., takes appropriate steps to assure that the patient is compliant with the prescribed plan of care. Participates in department performance improvement initiatives. Develop and maintains vascular access tracking data for Regional Quality Monitoring and Reports (i.e., monitoring of Optimal Starts on dialysis).
Requirements: Three (3) years of RN clinical nursing experience. Current Georgia RN License. Basic Life Support (BLS) Certification Valid Georgia driver's license. Working knowledge of vascular access anatomy and physical examination. Ability to diagnose a dysfunctional vascular access by physical examination. Knowledge of the techniques used for vascular access surveillance and monitoring. Knowledge of cannulation techniques including creation of buttonholes. Strong writing and communication skills required. Ability to read, analyze and interpret medical and scientific data. Ability to respond to common inquiries or complaints from physicians, staff, and patients.
