Pediatric Provider
to join us in the heart of Brevard County's Space Coast. Since 2005 our focus has been on putting the "community" in Community Health while delivering healthcare to more than 65,000 unique patients annually. We are expanding, we are growing. POSITION SUMMARY
To provide superior quality, competitive value, and outstanding service by providing the highest standard of quality medical care to Brevard Health Alliance (BHA) patients through the primary care medical home model. Under the observation of their supervising Physician and under the Chief Medical Officer, the APRN provides extraordinary patient care throughout BHA Clinics.
GENERAL EDUCATION REQUIREMENTS
Master of Science in Nursing from an educational institution approved by the Florida Department of Health.
ADDITIONAL QUALIFICATIONSClear and active Florida Advanced Practice Registered Nurse licensure3+ years' experience of related clinical experience preferred. Experience in a primary health care environment, community health center or Federally Qualified Health Center (FQHC) preferredExperience working in various outpatient settingsCurrent Basic Life Support (BLS)/CPR certification from the American Heart Association or the American Red CrossProficient in Electronic Medical Records (EMR)Demonstrates strong oral and written communication skillsAbility to speak and understand the English language to effectively communicate with patientsExtensive knowledge of physical assessment, differential diagnosis, pathophysiology
PRIMARY ACCOUNTABILITIESManages acute, episodic outpatient care and health maintenance needs through the provision of a Primary Care Medical HomeDemonstrates competent skills in therapeutic, cost-effective pharmacologic administration and managementDevelops and implements formal and informal teaching plans for patients, families, staff, and communityPromotes patient/family-informed decision making in treatment plans and optionsPerforms physicals, diagnostic tests, and screening evaluationsManages overall patient care regarding lifestyle and development issuesEmphasizes preventative care and disease managementUnderstands the changes in health promotion throughout the aging process, consults with the collaborating physician as warrantedCollaborates with Chief Medical Officer in development of clinical protocols/procedures for delivery of outpatient careProvides care to patients in a safe environmentParticipates in departmental Continuous Quality Improvement efforts and provides support for improvement of identified issuesWorks with clinical manager to ensure collaborative problem solving with staff through participation in identification, mediation, and resolution of issues and team problemsPromotes staff and community enhancement through coordination of in-services to staff, Physicians, and partners to promote profession and organizationAttends monthly department staff meetings, Provider meetings, and other scheduled meetingsMaintains Quality Department Performance measuresMaintains predetermined productivity based on patient volumesMaintains internal customer satisfactionTakes ownership and represents Brevard Health Alliance's mission and organizational goals established by the executive committee. These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate's Key Performance Indicators and will be evaluated on a monthly and quarterly basis.
