*Job Summary*We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will provide high-quality care to patients across various settings, including clinics, hospitals, and home community care. This role involves assessing patient needs, developing personalized treatment plans, and implementing therapeutic interventions to enhance mobility and overall physical function. The Physical Therapist will work with diverse populations, including geriatrics and pediatrics, ensuring that each patient receives tailored support throughout their recovery journey.
*Responsibilities** Conduct thorough assessments of patients’ physical abilities and limitations.* Develop individualized treatment plans based on assessment findings and patient goals.* Implement therapeutic exercises and modalities, including trigger point therapy, to alleviate pain and improve function.* Collaborate with healthcare teams in acute care settings and inpatient facilities to ensure comprehensive patient care.* Provide education to patients and their families regarding discharge planning and home exercise programs.* Maintain accurate documentation of patient progress and treatment outcomes in compliance with clinic standards.* Stay updated on the latest rehabilitation techniques and best practices in physical therapy.
*Experience** A degree in Physical Therapy from an accredited institution is required.* Previous experience in a clinical or hospital setting is preferred, with exposure to geriatrics, pediatrics, or acute care environments being advantageous.* Familiarity with discharge planning processes and community resources for ongoing patient support is beneficial.* Strong communication skills are essential for effective collaboration with patients, families, and multidisciplinary teams.* A commitment to professional development through continuing education in physical therapy practices is encouraged.

Pay: $43.00 - $45.00 per hour
