The Home Health Physical Therapist (PT, DPT, RPT)
is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patients well-being, and evaluates the patients progress.
Education RequirementsRecent graduates who have applied for the registration exam may be considered for provisionalLicense RequirementsMust be currently licensed to practice physical therapy in the state of practice.Current CPR certificationMust have a current drivers license and vehicle insurance, access to a dependable vehicle, or public transportation.Skill RequirementsMembership or certification in national association (as applicable)Must be willing to accept management duties asThis position can be employed by the individual Agency or be a contracted
