Your application is still being considered.With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.This position is located at the SC Dept. of Mental Health at Santee-Wateree Community Mental Health Center (CMHC), at the Camden Clinic, located at 2611 Liberty Hill Road, Camden, S.C. 29020, and will travel throughout the Santee-Wateree catchment area as needed.
Serves on an Assertive Community Treatment ( ACT) team providing evidenced-based services in an intensive non-residential treatment and rehabilitative services throughout the Santee-Wateree catchment area. This is a single fixed point of responsibility for treatment, rehabilitation, and support needs for patients with Serious Mental Illness (SMI) who require higher levels of community care and have not been well supported in lower level of care options.Provides direct services to ACT patients guiding them in the development of skills/strategics towards achieving and maintaining good mental health and facilitating training of independent living skills. Utilizes assertive engagement to engage patients in service. Perform all job duties and provide all services and documentation in compliance with CARF, SCDMH, SWCMHC, Quality Assurance, Compliance, HIPAA, Transcultural and Productivity Standards, Policies and Procedures.Facilitates the person-centered planning process for patients as assigned. Assists patients with access to community services, resources and agencies and establishing support networks in the community.Completes screenings and assessments for referrals and assists with development and revisions of instructional continuity plans (ICP). Provides family psychoeducation and works with patient's natural support.Participates in clinical supervision, treatment team meetings, pursues professional development, and demonstrates effective use of social and interpersonal knowledge and skills and demonstrates an understanding and appreciation of individual and group differences and similarities. Effectively utilizes and maintains certification of evidence-based practices. Attends treatment team meetings daily and provides consultation regarding complex cases as appropriate.
Minimum and Additional Requirements
State requirements : A Masters' degree in guidance, counseling or the social sciences or a related field.Internal requirements : Must be licensed or eligible for licensure in SC and willing to work towards licensure in SC within first six months of employment.**One year experience working with adults diagnosed with serious and persistent mental illness.Note: Applicants must also meet Agency minimum requirements to be considered for this position.Preferred Qualifications
Prefer candidates licensed as LPC, LISW, LMFT, LAC and/or CAC-I/II Certification.Bilingual abilities are a plus.Additional Comments
