100% FTE, days. Monday - Friday, eight-hour days between the hours of 8:00 am and 5:00 pm. No weekends, holidays, or on-call. You will work at East Madison Hospital in Madison, WI.

We are seeking a
Nurse Practitioner or Physician Assistant
to:Provide complex symptom management recommendations.Work in collaboration with the Palliative Care Medical Director, attending medical team, and Palliative Care Team members to deliver palliative care to patients in the acute care setting with life-limiting chronic diseases.Effectively communicate with patients and family by utilizing concepts of motivational interviewing.Perform assessment process to include patient history, physical, psychosocial, and spiritual assessment.Provide support, counseling, and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness.Facilitate patient/family conferences as related to palliative care or hospice.At UW Health, you will have

Master's or Doctorate's degree from accredited Nursing Program.If Physician Assistant:
Bachelor's or Master's degree from accredited PA Program.
Licenses and Certifications :Minimum - State of Wisconsin NP or PA licensureIf Nurse Practitioner*:
Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice ANDActive board certification as a NP in area of clinical practice. These may include the following: Adult; Adult/Gerontology Acute Care; Adult/Gerontology Primary CareIf Physician Assistant*:
Active PA license, including prescriptive authority, in state(s) of practice ANDActive PA board certification
