**Must-Have:**- Experience with Home Care Home Base- Infusion and Wound care skills- Proficiency in Oasis (preferred)
**Job Description:**
- Implement a full-time professional nursing service program for patients in their homes, under the supervision of the Clinical Services Supervisor, in adherence to agency standards and policies.
- Initiate and provide medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition, the achievement of rehabilitation potential, and/or the reduction of pain and other symptoms.
- Conduct initial visits to assess the patient's physical and emotional status, needs, and resources through observation, interviews, and analysis of records.
- Prepare the nursing plan of care, in accordance with the physician's plan of treatment, based on problems identified in the nursing assessment, and provide patient teaching using the Care Plan/Standard of Care.
