FlexStaff Our client is seeking a dedicated and experienced RN Care Delivery Field Supervisor to join their team. Reporting to the Clinical / Center Service Director, the RN Care Delivery Field Supervisor will oversee and ensure high-quality clinical services for patients in their homes. This role is integral to maintaining our nursing standards of excellence and delivering care that is sensitive to diverse backgrounds and needs.Make a difference in the lives of the geriatric population by ensuring they receive high-quality home health care. Key Responsibilities: Supervision & Delegation: Collaborate with the Care Delivery Manager to supervise and delegate responsibilities to assigned personnel, ensuring quality care and adherence to care plans. Assessment & Evaluation: Assess and evaluate participants' health care needs, considering physical, mental, and environmental factors. Guidance & Instruction: Supervise and guide Community Health Nurses (CHNs) and Home Health Aides (HHAs) in delivering quality home health care services. Joint Visits & Competencies: Conduct joint visits with CHNs for supervision and competency assessment. Orientation & Training: Assist in designing orientation programs for new employees and provide education to participants and aides on medications, safety, and diet. Collaboration & Care Planning: Work with the interdisciplinary team to develop comprehensive care plans tailored to individual participant needs. Documentation & Reporting: Maintain accurate documentation and submit reports as required by Health System standards and federal/state regulations. Field Visits & Audits: Conduct field visits in place of CHNs as needed and perform routine chart audits. Travel: Travel is required, including potential coverage of neighboring sites. Regulatory Compliance: Act as a core interdisciplinary team member, ensuring compliance with CMS PACE regulatory requirements. Qualifications: Education: Degree from an NLN-accredited nursing school (RN, BSN required). Baccalaureate degree highly desired. At least five (5) years of experience in a healthcare setting is acceptable in lieu of BSN. Experience: Minimum of one (1) year of experience as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program, or Licensed Agency. Experience working with geriatric populations preferred. Experience in a PACE Program or MLTC preferred. Managed long-term care insurance experience beneficial. Skills: Excellent organizational and presentation skills. Proficient with computers and laptops. Strong knowledge of New York State Department of Health regulations. Licensure: Current active and unrestricted license and registration in New York State as a Registered Nurse required.
