A healthcare network in New York City is looking to add a licensed Registered Nurse (RN) to their Hospice team in the Bronx. In this role, the Hospice Nurse (RN) will be responsible for visiting patients in a community setting and providing palliative care services, as needed.

Perform both physical and psychosocial assessmentsElectronically document visitsDevelop and implement nursing plans of careMake recommendations for care based on patients' needsContact the patient's Physician to request written orders to facilitate plan of care changesSupervise, train and evaluate Home Health Aides and homemakers working with hospice patientsPerform other duties, as neededQualifications:
Previous hospice or 1+ years of experience in a hospital settingGraduate of accredited School of NursingNYS Registered Nurse (RN) licenseDesired Skills:
