A healthcare network in New York City is looking to add a licensed Registered Nurse (RN) to their Hospice team in Manhattan. In this role, the Hospice Nurse (RN) will be responsible for visiting patients both community and facility settings and providing hospice services, as needed. About the Opportunity: Specialty: Hospice Patient Population: Adults / Geriatrics Schedule: Monday to Friday Hours: Standard business Call: Yes Responsibilities: The Hospice Nurse (RN) will: Perform both physical and psychosocial assessments Electronically document visits Develop and implement nursing plans of care Make recommendations for care based on patients' needs Contact the patient's Physician to request written orders to facilitate plan of care changes Supervise, train and evaluate Home Health Aides and homemakers working with hospice patients Perform other duties, as needed Qualifications: 2+ years of experience in a Hospital setting Graduate of accredited School of Nursing NYS Registered Nurse (RN) license Desired Skills: Bilingual Hospice and/or Home Care background
