Job Description
The Physical Therapist is responsible for developing a physical plan of care based on a complete assessment and the direction of the patient's physician, integrating the physical therapy into the total Home Health plan of care and coordinating services with other team members involved in the plan of care. The Therapist may be requested to evaluate Hospice patients for palliation of symptoms that restrict mobility, transfers and ADLs.

Additional Information
Wage:
Wage range currently starts at $47.38/hour and goes up to $71.81/hour. Actual wage will be based on years' of applicable experience. This position may also be eligible to receive an additional 15% premium in lieu of benefits.
