Overview
Physical TherapistFull TimeRock IslandFunction as a member of the interdisciplinary team. Perform Physical Therapy Evaluations. Establish Physical Therapy treatment plans. Treat patients in a variety of settings including: Acute Care, Transitional Care Unit, IP Acute Rehab, NSCU. Patient populations served include pediatric, adolescent, adult, geriatric. Provide patient/family education. Perform appropriate documentation in electronic medical record. Participate in patient care conferences/ staffing as appropriate. Supervise support personnel including Physical Therapist Assistants and Rehab Aides.Responsibilities
Clinical Excellence/Patient CarePerform Physical Therapy evaluation and development of the plan of care appropriate for each patient based on the patient's physical, mental, emotional, and developmental impairments and functional limitations. A variety of diagnoses and settings served include but are not limited to Acute Care, Transitional Care Unit, IP Acute Rehab, and NSCU.Provide Physical Therapy interventions in accordance with established standards of care, policies, procedures, and state/federal law.Supervise Physical Therapist Assistant and Rehab Aides.Supervise and complete appropriate documentation for Physical Therapy and Physical Therapy Assistant students as assigned by CCCE.Complete documentation in electronic medical record in a timely manner in accordance within department and professional expectations demonstrating the need for skilled interventionsParticipate in patient care conferences/ staffing as appropriate. Acute care- Complex Care meeting. Transitional Care Unit- Patient staffing. IP Acute Rehab- Patient staffing.Contribute to department and hospital process improvement.
Qualifications
Education:Certificate, Bachelor's degree, Master's degree, or Doctorate degree from an accredited Physical Therapy program.License(s)/Certification(s):Current IA and IL Physical Therapy licensure (or eligible).Current CPR certification.Why UnityPoint Health?Commitment to our Team -
