At St. Lukes, our dedicated team of RNs strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.St. Lukes home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patients home. From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life. Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care. Our home-based services team (NP, RN, MA, MSW) also assists with oversighting transitions. Close working relationship with St. Lukes Hospice team.The Home Care Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care from admission to discharge for an assigned case load. This is a direct care position providing in home care that requires traveling within the Treasure Valley area.Why Home Health"

QualificationsEducation: ASN or BSN DegreeExperience: 2 years of RN experience; Hospice or Acute Care experience helpfulLicenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) CertificationValid Driver's License and proof of Auto Insurance
