Overview
Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). Our Mission is to promote health and wellness through excellent, compassionate care for all.
Under the supervision of the Referral Manager, the Referral Specialist manages the in-house specialty clinics and works as a liaison for all specialists. The Referral Specialist is responsible for coordinating the specialty clinics by managing the waiting lists, scheduling the patients, and working on follow-up items after the clinics take place. The Referral Specialist works closely with the Referral Navigators, the Specialists and Medical Scribes and seeks guidance from nurses and providers as needed for clinical resources. This position provides extensive support to providers and patients.
Responsibilities
* Processes, tracks and closes both internal specialty referrals and outside specialty referrals.* Meet with other community clinics on a regular basis to inform them about MCC's clinics that are open to outside referrals.* Works as a specialist liaison and address specialists' inquiries and concerns with the assistance of the Referral Navigator Manager.* Manages the waiting lists for in-house specialty clinics.* Ensures that the protocol for each specialty clinic has been followed.* Schedules the appointments for the patients.* Sends letters to the patients with the appointments' information.* Makes reminder calls 48 hours before some of the clinics and remind the patients of what they need to bring to their appointments.* Reviews doctors' notes from clinics and follows up on all requests- schedules follow up appointments, refers patients to other programs, work on imaging requests, surgery requests etc.* Maintains accurate records of specialist appointments that are kept, rescheduled or no-shows in referral tracking system. Reschedule patients if needed. Notifies the PCP if the patient is non-compliant or the appointment is no longer needed.* Creates workflows for medical assistants and specialists when needed. Provide training to medical assistants and call center staff.* Maintains ongoing tracking and documentation of referrals in EMR.* Ensures that referrals are obtained in a timely manner and communicate with clinical staff regarding any problems scheduling patients.* Per referral guidelines, provide appropriate and complete clinical information to specialist.* Contacts patient regarding any pending referrals or needed diagnostic testing or labs.* Responds to patient requests and concerns regarding in-house specialist visits.* Monitors in-house specialty clinics no-show rates. Investigate reason for no-shows and propose changes when appropriate.* Submits and tracks charity application to hospitals and keep providers and patients updated.* Other duties as assigned.
Qualifications
Education & Experience:
* High school diploma or equivalent (GED) required.* College education highly desired.* Ability to read and interpret medical records in English.* Previous experience creating work-flows and providing training.
Required Skills and Abilities:
* Knowledge of medical terminology a plus.* Excellent written and verbal communication ability.* Ability to work and communicate with patients and representatives of other organizations via phone.* Bilingual in Spanish is required.* Must be sensitive to multi-cultural, multi-ethnic backgrounds.* Ability to work in team environment office setting.* Strong customer service experience and abilities.* Strong attention to detail ability.
Physical Requirements and Working Conditions:
* Prolonged standing and walking.* Prolonged periods of sitting at a desk and working on a computer.* Must be able to lift up to 15 pounds at times.
