Our client, a community-based healthcare center providing culturally competent, community-based care and addressing the special healthcare needs of underserved, low-income New Yorkers located in New York City, is hiring an Ambulatory Primary Care Nurse Practitioner. Join a team dedicated to providing programs and services that include primary and preventative care, gynecological care, reproductive health services, cardiology, podiatry, psychiatry, audiology, ophthalmology, physician house calls and physical/occupational therapy as well as comprehensive HIV care. They are an FQHC (National Health Service Corps) and all of their providers are entitled to the tuition waiver.
Responsibilities
Provide evidence-based Primary Care to our patientsBe an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.Work with a dynamic team providing care to medically underserved communities throughout New York City.
Requirements
Graduation from a professional degree program as a Nurse PractitionerCurrent license as a APN in NYSDEA licenseValid Family Nurse Practitioner N.Y.S. License to deliver medical carepost-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
