Job Title

: Physical Therapist PACE

Job Summary

The Physical Therapist evaluates and reassesses participants to assess mobility level and other functional abilities. Provides and recommends therapeutic exercise, gait training, ambulation, etc. Plans and develops group exercise programs for maintenance therapy for participants. Responsible for the overall Physical Therapy program for Florida PACE Centers .

Essential Job Functions

Evaluates, identifies, and provides therapeutic interventions as indicated in the care planDevelops an appropriate and comprehensive treatment plan for each participant in order to maximize the participant's potentialReviews participant's status at regular intervals and changes are made to treatment program as necessaryCompletes assessments and discharge summariesPlans and develops maintenance, exercise, and therapy programs for participantsProvides supervision to program aides and other staff who are implementing a Physical Therapy treatment programParticipates in and provides in-service education to facilities, supportive staff, students, and the communityDocuments all treatments rendered and maintain appropriate work recordsActively participates as a member of the Multidisciplinary Team, and participates in case conferencesJob Requirements

Education:

Graduate from an accredited school of Physical Therapy approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association or other equivalent organizations approved by CMSExperience:

One year experience of physical therapy experience working with frail or elderly populationLicenses/Certifications:

Florida State License as a Physical TherapistAbilities Required

Current knowledge of safety issues needed in the treatment of participants, especially regarding body mechanics and body substance precautions to protect self, participants and personnel working with the therapists must be maintainedDemonstrated human relations and effective communication skillsBilingual in English/Spanish required, Creole a plusFunctional Demands
