As an Outreach & Rapid Response Coordinator, you'll be on the front lines, connecting with chronically homeless individuals, particularly those recently released from incarceration. You'll work closely with community partners, law enforcement, and social service agencies to provide essential support and resources.
Key Responsibilities: Street Outreach:
Build rapport with individuals experiencing homelessness, offering support, guidance, and resources. Case Management:
Develop and implement comprehensive case plans, addressing housing, healthcare, and other critical needs. Collaboration:
Work closely with a network of agencies to ensure seamless coordination of services. Crisis Intervention:
Respond to immediate crises and provide support to individuals in distress. Data Collection and Reporting:
Maintain accurate records and generate reports to track progress and inform program decisions. Qualifications:
Must be in active recovery for min of 2 years. Passion for helping others and a strong commitment to social justice. Experience working with individuals experiencing homelessness, mental health challenges, or substance abuse issues. Excellent communication and interpersonal skills. Strong organizational and time management abilities. Ability to work independently and as part of a team. Valid driver's license and reliable transportation.
