The Community Response Case Manager will play a vital role in providing support to individuals experiencing homelessness and utilizing emergency city services. This position will work closely with the OKC Fire and Police Department, City of OKC, and the Street Outreach Crisis Response Team Lead to identify and address the needs of these individuals.

Monday - Friday
8:30am - 5:00pm
Key Responsibilities:
- Case Management: Review referrals, conduct outreach, assess needs, develop goals, and provide ongoing support to individuals.
- Coordination: Collaborate with partner agencies to ensure participants receive necessary services like mental health, housing, and legal assistance.
- Documentation: Maintain accurate records and data to monitor progress and support program development.
- Outreach: Participate in outreach activities to build relationships with individuals experiencing homelessness.
- Bachelor's degree in social work or a related field preferred.
- Must be in active recovery for a minimum of 2 years.
- Excellent organizational and time management skills.
- Proficiency in Microsoft Office Suite.
- Strong communication skills, both verbal and written.
- Ability to work effectively in a multidisciplinary team.
- Valid Oklahoma Driver's License.
- Combination of office and field work, including outreach to encampments.
- Exposure to various weather conditions and challenging environments.
- Potential for exposure to stressful and physically challenging situations.
- Flexible work schedule and occasional travel may be required.
- Ability to lift and carry 25 pounds, partner lift 50 pounds.
- Ability to stand and walk for extended periods.
- Physical mobility and endurance to navigate various environments.
