POSITION SUMMARY: Carries out the services of the supported employment program by assisting clients in obtaining and maintaining employment that is consistent with their vocational goals and by establishing working relationships with community employers.
RESPONSIBILITIES:
Duties on behalf of clients include: employment development, vocational assessments,
job preparation, job coaching, vocational counseling, environmental support, adaptive skills teaching. A substantial amount of time will be spent in the community setting meeting with clients and employers. Transports clients as needed in own automobile. Performs other duties as assigned.
· Provide Vocational Support Services, including but not limited to: Job coaching and travel training, evaluation, including functional assessment, guidance, and referral to resources.
· Prepare Functional Assessments of clients to identify strengths in relation to current skills, learning ability, work history, and social skills.
· Provide assistance and encouragement to clients to explore vocational goals, strengths, and problems. Develop resumes.
· Provides job seeking skills training.
· Provides services for OOD as supervised by the Employment Services Team Leader.
· Participate in the creation and completion of Individualized Service Plans (ISPs) with and for clients, which set forth vocational goals, interventions, and performance measures.
· Introduces clients to Ohio Means Jobs and other community resources.
· Schedule, coordinate, and obtain employment opportunities for clients.
Schedule job interviews for clients
Accompany client to job interviews (based upon client preference).
· Provide guidance to clients concerning benefits i.e., basic Social Security information, work incentives, special rules etc.
· Provide follow up contacts and services with clients to maintain employment.
Job Development Duties:
· Carry out job development activities with community employers to secure employment opportunities for clients.
· Market employment services to community employers, during which time benefits of the employing clients are explained.
· Provide ongoing service to hiring employers to maintain client employment.
· Maintain collaboration with other agencies.
Other Responsibilities:
· Maintain records of information, such as:
· Maintain safety, security and confidentiality of clients.
· Attend supported employment monthly team meetings, treatment team meetings, and other meetings as required or assigned.
· Participates in staff development and training as required or assigned.
· Other duties as assigned by supervisor and ALLWELL Administration.
Will have access to and use of, as well as may create, Protected Health Information (PHI) of clients through verbal, written and electronic communications, as recorded and contained in ABHS's clinical records and electronic healthcare record system. Will use and protect such identified client PHI by maintaining and respecting client privacy and confidentiality in conformance to federal HIPAA and state patient privacy laws and regulations and ABHS client confidentiality policies. Employee will only access and use the minimum necessary PHI required to perform their job duties.
QUALIFICATIONS:
· Associate Degree or equivalent with one year experience in the mental health and/or vocational rehabilitation field. Those experienced in sales will be considered.
· Must possess good interpersonal communication skills, organizational skills, case documentation skills and computer skills.
· Provides complex explanations to others, verbally and in writing that rely on technical knowledge and thought.
· Adheres to client confidentiality.
· Will be expected to be certified in CPR, and first aid within six month
