The Oncology Nurse Navigator (ONN) uses advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. The ONN offers individualized support across the continuum of care to patients, families, and caregivers to expedite care delivery and alleviate barriers. The ONN is responsible for the coordination of care through interdisciplinary collaboration with a a goal of delivering efficient care with optimal outcomes. The ONN will ensure ease of transition through various health settings during the patient's cancer journey. Facilitates daily coordination of patient care. Interacts with staff, patients, families, visitors, members of the health care team and external partners to develop, coordinate, and manage the plan of patient care. Must work autonomously and in a team environment. Utilize delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care. Demonstrate the role of patient advocate, protector of patient's rights, provider of psychological/social/cultural support, and tolerate stressful situations. Excellent interpersonal communication, customer service and teamwork skills required. Above average organizational skills, ability to multi-task, and attention to detail in work. Strong skill in written and verbal communication. Organizes, formulates and coordinates a multi-disciplinary approach to the patient population for achievement of optimum outcomes. Ability to clinically triage new patient referrals efficiently to ensure timely access to care. Assesses, implements, and coordinates patient care plans including, but not limited to assess referrals to ensure appropriateness of admission/appointment and criteria is met; screening of referrals to ensure appropriate patient placement in programs; ensuring patients and/or family members understand the plan of care; preparing the patient and family for ongoing care; and responding to patients' needs and concerns. Collaborate with multidisciplinary care team and expedite sequence of patient's workup, active treatment, survivorship, and end of life care. Targeted symptom assessment and management for identified patient population. Provides communication, advocacy, and education in a culturally competent manner. Assess and document patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery. Enhance access to therapeutic clinical trials and optimize communication with patients, families and investigators regarding therapeutic clinical trials, including patient-specific education about clinical trials. Plans, implements and coordinates strategies through evidence-based practices to support quality patient care; and improve outcomes. Serves as a liaison between department staff, patients and families, physicians, external vendors/partners to ensure a collaborative approach to care for patients and families. Possess leadership skills including staff education, preceptorship, patient education, and participate in process improvement initiatives. Encourage and empower patients to actively participate in decision-making about their treatment and care. Serves as advocate for patient, protector of patient's rights, and provider of psychological/social/cultural support. What We Require: Education:
Graduate from an accredited school of nursing BSN/MSN preferred.
Experience:
Three years of experience of specialty specific nursing in a hospital, clinic, or community setting. National Nurse Navigation training preferred.
Licensure/Certifications/Registration:
Current licensure as a Registered Nurse in the State of Virginia required. AHA BLS-HCP required.
Other Minimum Qualifications:
Excellent written and oral communication skills. Excellent interpersonal skills, organizational ability, and initiative. Proven ability to understand medical terminology, problem solving and handle stressful situation with a professional manner.
