San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.

This is a full time position in our El Cajon Clinic. Typical hours are 8:00AM - 4:30PM, Monday - Friday.Position Summary:

Provides primary medical management in collaboration with other St. Paul’s PACE Physician(s) and providers. Performs initial and semi-annual assessments and performs additional physical assessments of participants as needed and develops and implements appropriate plans of care to St. Paul’s PACE program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. At the direction of the Medical Director functionsas a member of the Interdisciplinary Team (IDT). Rounds in the St. Paul’s PACE contracted nursing homes, Assisted Living facilities, Board and Care facilities and participants homes.. At the Direction of the Medical Director informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant’s caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies.Basic Qualifications:Education/Training/Certifications:1. Certification as a MD, with the appropriate perspective authority2.
