Job Description:- Provide occupational therapy services to patients in their residences who have cognitive and/or physical impairments resulting from injury, disease, disability, or other causes.- Collaborate with patients, families, physicians, and other health professionals to determine patient needs and coordinate services.- Develop plans of care that are patient and family-focused, and implement standardized interventions for goal-oriented outcomes.- Conduct daily documentation in a computerized system accurately reflecting objectivity and measurability.- Apply standard treatment procedures and provide teaching to patients and caregivers.- Maintain regular interdisciplinary communication.- Reassess and modify the plan of treatment every two weeks in alignment with patient, caregiver, and team goals.- Ensure physician orders are current and align with the plan of care.- Submit patient schedules daily.- Orient and supervise home health aides according to agency and state guidelines.- Manage caseloads for Occupational Therapy Assistants in compliance with agency, state, and professional guidelines.- Complete documentation and submit it punctually, adhering to agency standards.- Verify payor information at admission according to agency standards.- Support managed care authorizations and DME requirements documentation within specified time frames.- Plan visit schedules consistent with authorizations from managed care payors.- Identify realistic, objective, time-oriented goals for patient performance based on examination findings.- Discuss utilization and caseload with management as needed and make necessary adjustments to meet goals.- Select treatment and discharge plans based on examination findings and patient goals.- Attend regular staff meetings and educational in-services.- Complete clinical competencies and mandatory training as required.- Inform patients of their weekly schedule and any changes as necessary.
