Outpatient Geriatric Medicine Opportunity in Pinebrook/Mobile, AL

Position Summary:
Join our collaborative care team dedicated to delivering exceptional geriatric medicine in Pinebrook/Mobile, AL. Our multidisciplinary team includes Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, Referral Coordinators, and more. This comprehensive support structure empowers our primary care physicians to reduce their patient load, allowing for more meaningful patient interactions and care.
Primary Responsibilities:
- Evaluate and treat patients in accordance with established standards of care.
- Coordinate patient services, including specialty referrals, hospital and skilled nursing facility (SNF) coordination, durable medical equipment, and home health care.
- Actively engage with the care team during daily huddles, providing medical expertise and input.
- Collaborate with the Lead Physician and Center Administrator to foster a cooperative and professional practice environment.
- Complete daily medical record documentation, partnering with quality-focused coders to ensure comprehensive documentation.
- Adhere to policies and protocols set forth by Clinical Leadership.
- Meet regularly with the Lead Physician to discuss quality of care, review outcome data, and address medical record issues.
- Maintain a focus on direct patient care, balancing patient-facing time with necessary administrative responsibilities (meetings, etc.).
Required Qualifications:
- Graduate of an accredited MD or DO program.
- Current, unrestricted medical license or willingness to obtain a medical license in Alabama; eligibility and willingness to secure licenses in other states as needed.
- Board Certification or eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Strong interpersonal skills with the ability to communicate effectively with colleagues and patients.
- Commitment to team-based care principles.
- Openness to learning and adapting to a value-based care model.
- Exceptional patient and customer service skills.
- Basic computer proficiency, including email and electronic medical records (EMR).
Preferred Qualifications:
- Familiarity with HEDIS/STARS metrics.
- Understanding of CAHPS and HOS surveys.
- Experience in medically underserved and culturally diverse communities.
- Proficiency with electronic health records (EHR).
- Experience with Medicare Advantage Global Risk Models.
