Workdays: Monday - FridayWorks Hours: 11:30 a.m. - 5:00 p.m.Location: 1115 Mission Rd., San Antonio, TX. 78210

Mission:

Seton Home works to break the cycle of abuse and poverty by providing a caring home, education, and support services necessary to transform the lives of pregnant and parenting teen mothers and their children.

Position Responsibilities:

Medication Logs and Prescriptions:

*Responsible for scheduling required medical and dental exams, necessary follow-ups and appointments requested by residents within allotted timeframes in accordance with TDFPS Minimum Standards.*Manage and monitor the delivery of services to youth ensuring compliance in all aspects of performance in accordance with Seton Home's Policy and Procedures and TDFPS minimum standards.Ensure medical incident reports are completed and signed for all medical events involving residents and their children and conduct or coordinate follow-up with necessary personnel.Ensure staff is notified timely of medical recommendations to be followed during ER, Hospital Visits, or routine follow-ups.Process incoming prescribed medication and discard of expired and/or discontinued medication, in compliance with TDFPS Minimum Standards and licensing requirements.Ensure medication is properly labeled and stored in designated areas.Responsible for maintaining OTC medication and medical supply inventory in coordination with Resource and Activity Coordinator.Conduct daily medication administration log audits and ensure that medications are being administered properly per the prescribing physician's orders and notify Program Director of any errors to be addressed.Contact pharmacy for information on prescriptions in a timely manner and ensure a 14-day supply is maintained on all prescribed medications at all times.Train staff on the Seton Home Medication Administration Procedures & Process as requested by Shift Supervisor(s).Coordinate with Case Manager(s) to conduct medical consenter training and psychotropic medication training for residents aged 16+.Ensure compliance and expectations of all licensing, contractor and grantors are met.Update youth's records in regard to allergies, medication use and immunizations history after each visit.Program Duties include but not limited to:

Participate and assist in the development of resident Plans of Service in coordination with the Program team every 60 days.Attend medical appointments and psychotropic medication reviews with residents.daily schedule is subject to change based on appointment days/times.*Educate and counsel youth concerning their diagnosis, treatment and prevention.*Maintain current knowledge of all TDFPS and other contract provider policies and procedures related to the medical care of the youth.* Teach Prenatal, Labor & Delivery, Post-Partum and Breastfeeding lessons to residents as needed.*Demonstrate proficiency in knowledge of CPR/First Aid, Emergency Behavior Intervention and child abuse reporting policies and procedures.Administrative Duties include but not limited to:

Provide information regarding to scheduled resident appointments via team's calendar and collaborate with Shift Supervisor(s) to ensure resident(s) are availableComplete incident reports promptly as outlined in the Minimum Standards.*Compile and maintain medical files on each resident and their child. Ensure proper documentation is uploaded into Extended Reach; maintain a hard copy of required documents and medical records in resident's file.*Collect and maintain medical data to be submitted for program reports and resident's Plans of Service*Actively participate in all quality assurance meetings to ensure the quality care and accurate and informed services by all team members.Serve as a role model in carrying out Seton Home's Mission.

Must be sensitive to the service population's cultural and socioeconomic characteristics.Adhering to safety training and protocols on a daily basis and taking precautionary measure to ensure the safety and well-being of self, others.Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information.Adherence to the Code of Conduct and the Faith and Moral Policy is mandatory.Every employee is required to take a solution-oriented approach in their interactions and undertakings, as well as being a team member that promotes collaboration and commitment to the Mission and Vision of the organization.As a Seton Home employee, attendance and successful completion of New Employee Orientation and Training is mandated for the position. The inability to meet this requirement will conclude employment with the Agency.Other duties as assigned by Program Director or VP of Programs.

Minimum Qualifications:

Education

Associate's Degree in relevant field preferred.CPR Certification PreferredMust be willing to be trained in Prenatal, Labor & Delivery, Post-Partum and Breastfeeding within first 90 days

Experience

Minimum of 1 year experience in adolescent pregnancy and/or pediatric setting.

License and Credentials

Medical Coordinator experience preferredValid driver licenseValid vehicle insurance

Minimum Knowledge and Skills:

Must be detail oriented, organized, self-motivated, work well independently and on a team.Must have good written and verbal skills.Must have good critical thinking and problem-solving skills.Ability to work under pressure to meet organizational needs and other deadlines.Ability and willingness to work some evening and/or weekend hours as needed.

Travel Requirements:

Travel requirements for the position includes 20% local and 0% overnight.
