AHA or ARC OnlyProof of vaccine and 1 booster or exemptions must be submitted for approval.Proof of 2 immunizations, positive IGG titer, if negative titer, must have booster or Declination accepted.Will be completed onsite with CHS on start dateProof of 3 immunizations and positive antibody titer, if negative titer, booster or Declination accepted.Required from October - March (unless otherwise indicated by CDPH) Medical/Religious exemptions will be sent for approval with a physician's statement(medical) or written statement from traveler(religious).Annual negative TB skin test or QuantiFERON Gold or T-Spot. Must state negative results. If positive, must show proof of positive history, initial Chest x-ray and annual TB Questionnaire thereafter.Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.Proof of 2 immunizations, positive titer or Declination accepted.Proof of vaccination within past 10 years or Declination accepted.Required on start date for clinical staffResume or Profile2 Professional References, 1 minimum Supervisory7 year County, National Criminal Background Check, SSN Trace, Employment Verification, and VSOP completed within 6 months of first assignment. Updated every 3 years thereafter. If break in service > 90 days, must run counties listed during break in service.A diploma, degree or transcripts is acceptable. If applicable, board certified, primary source verification of highest level of education prior to first assignment and upon requalificationMust be manual search within 30 days of start and updated annually.CA state healthcare exclusion list. Medi-Cal Program eligibility completed within 30 days prior to first assignment and monthly thereafter.Must be within 30 days of start, 10 panel standard test. Updated annually. Rapid Drug screen accepted. *Will accept within 40 days if the start date is postponed. If Break in service > 30 days, retesting required.Clinical Only: Skill specific required to be complete within 1 year of start.Attestation of any known record of DNS/DNR for the candidate from prior 3 years on your agency’s letterhead.HLC Testing to be completed by Friday before Start.
