Position Summary

Population Health Specialist- The Population Health Specialist is a required GME position that is critical to the ongoing accreditation of the Family Medicine Residency Program through ensuring compliance with accrediting body ( ACGME )-defined requirements of the Family Medicine Practice, which is the foundation for Family Medicine Resident education and a platform to address health needs in the community where the practice is located.

Minimum Qualifications

EDUCATION :

Minimum of an associate degree in business administration, nursing, or related field.

EXPERIENCE :

Intermediate skills using Microsoft Office along with database software experience, Quest, Sharepoint, DropBox.

Preferred Qualifications

Bachelor’s degree

Experience working in an ambulatory setting.

Job Duties

Description of Job Duty

Job Responsibilities:

Run Quality Improvement initiatives in the clinic in collaboration with other staff

Conduct follow-up calls to patients regarding referrals

Obtain progress notes from referral organizations as needed

Obtain Prior Authorizations and insurance approvals for procedures done in clinic

Schedule radiology appointments as needed

Provide medical records to patients and outside providers as requested, following institutional policies regarding the release of medical records

Participate in annual strategic planning for the clinic

Assist providers with registration and renumeration processes for attendance at medical conferences

Plan and maintain on-call schedules for providers

Manage invoices for the clinic

Participate in meetings related to clinic finances

Input new vendors into invoice management system

Assist with other administrative duties and tasks as assigned

Immunization Requirements

It is MSM’s Immunization policy that all Prospective Employees are required to provide proof that they are vaccinated against COVID -19 prior to commencement of employment. If employment will commence during Flu Season, MSM requires all individuals to provide proof that they are vaccinated against Seasonal Influenza for the current Flu Season, unless granted an exemption. Failure to provide proof of vaccination for any required vaccines or obtain an exemption from MSM will result in rescission of a candidate’s offer of employment or disciplinary action up to and including termination.
