Overview

Under the Director of Admissions, the primary purpose of this position is to coordinate, evaluate and facilitate referrals for potential admissions to Rutland Nursing Home (RNH) and Schulman & Schachne for Nursing & Rehabilitation (SSI). Will serve as a liaison to the nursing homes to help ensure a smooth transition with the admissions process between staff and applicants, hospitals and community agencies.

Responsibilities

Reviews Patient Review Instruments (PRIs) and Screens and all supporting clinical documentation for appropriateness to be admitted to either RNH or SSI and coordinates all components of the admissions process with the interdisciplinary team.Will consult with hospital staff to review and evaluate potential applicants for admission to the facilities.Completes all required clinical pre-admission assessment forms on all potential admissions and gather additional medical and social information as needed for a thorough assessment.Review/analyze census reports for discharges, admissions, hospitalizations, and readmissions with follow up as necessary.Documents all applicant clinical and psycho-social assessments.Verification of payer source, i.e., Medicare, managed care, Medicaid, insurance.Interacts with HMO Case Managers prior to admissions to review skilled needs and appropriate level.Obtains authorizations for managed care organizations as needed.Weekend and holiday admission coverage as needed.Ability to navigate multiple referral systems which includes both internal and external referral platforms.Assist potential residents and their families by coordinating tours of the facilities and answer questions regarding the facilities and admissions process.Maintains accurate documentation on all applicants evaluated in accordance with State and Federal regulations.Assists other departments with projects as needed in the spirit of teamwork.All other duties as requested.Qualifications

Education:

Graduate of Accredited Nursing Program(s).Experience:

Previous experience in post-acute admissions processes required.Knowledge and Skills:

Excellent writing and clinical assessment skillsGood working relationships with staff and referral sourcesAbility to multi-task and work accurately in a fast-paced environmentBilingual (English/Spanish) a plusThorough knowledge and understanding of medical terminology, conditions and treatments relevant to geriatric, young adult and pediatric populationsAbility to make thorough and accurate bio-psychosocial assessments of geriatric, young adult and pediatric applicants in relation to the continuum of services provide by RNH and SSIPositive relationship buildingStrong interpersonal and verbal communication skills.Organized with an ability to work independently to complete projectsDetail oriented with consistent follow-throughExcellent listening and problem-solving skillsProficient with Microsoft Office suite applications such as Word and ExcelLicenses, Certifications:

New York State Registered Nurse or Licensed Practical NursePRI certified preferredPhysical Requirements:

Ability to work in a variety of office settings.Position requires prolonged periods of sitting, reaching, walking throughout the work day.Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
