We are currently looking for a Physical Therapist to join our team. The position would be responsible for
providing physical therapy evaluation and treatment to patients in their homes or alternate community settings.

ResponsibilitiesSupports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
Assesses and evaluates client for physical therapy needs and develops and implements appropriate plan of action in collaboration with the physician
Documents all changes in client’s condition and updates care plans and outcomes accordingly
Identifies counseling needs and plans appropriate teaching strategies
Monitors quality and appropriateness of services provided
Adheres to agency infection control and safety policies
Adheres to the organization’s policy in regards to absenteeism and appearance
Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
QualificationsPhysical Therapy Licensure in NJ required
2-3 years of prior clinical experience and community based experience preferred
Strong computer skills
Strong communication and interpersonal skills
Valid NJ drivers license
Working Conditions/Physical Demand : Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation
