About this position:The Care Coordination Registered Nurse evaluates timeliness and appropriateness of patient care, and coordinates discharge or transition planning for a smooth progression to post-hospital care.What you will need:Licensed to practice as a Registered Nurse in the State of Maryland. Graduate of an accredited Nursing program.Bachelor's Degree preferred or an equivalent combination of education and experience.Minimum of 2 years clinical experience in acute care setting along with one or more years' experience in Care Coordination preferred performing discharge planning from the inpatient or skilled facility setting.

Salary Range: Minimum 53.02 per hour - Maximum 53.02 per hour.
