Our programs include outpatient therapy, psychiatric care, substance abuse services, and Assertive Community Treatment (ACT) for children and adults.Position OverviewWe are seeking a full-time ACT Nurse (RN or LPN) to join our Assertive Community Treatment (ACT) team. The ACT Nurse plays a critical role in delivering comprehensive, community-based clinical services to clients with severe mental health needs. This position involves medication monitoring, crisis assessment, care coordination, and a range of therapeutic interventions to support clients in achieving stability and improved quality of life.Location: Charlotte, NCCompensation:LPN: $25.02 - $27.69 per hourRN: $72,666 - $80,014 annually (based on experience)Key ResponsibilitiesClinical Care:Monitor and document medical and clinical aspects of care for all clients receiving ACT team services.Administer and oversee psychopharmacologic and medical services within the scope of practice.Provide symptom assessment and management, crisis intervention, and individual counseling.Care Coordination:Collaborate with primary healthcare providers, inpatient facilities, and other medical professionals to address clients' holistic health needs.Coordinate with the ACT team to track clients' clinical status and response to treatment.Support Services:Facilitate activities of daily living, social skills, and leisure activities to enhance clients' independence.Provide education, support, and consultation to clients' families and natural supports.Documentation and Planning:Develop and maintain treatment and goal plans, including required medical record documentation.Assist with creating transition plans for clients moving in or out of services.Team Collaboration:Participate in crisis phone/on-call duties as part of a team rotation.Work with the ACT team to plan, schedule, and implement tailored services for clients.QualificationsLicensure:Must hold an active nursing license (RN or LPN) in North Carolina, issued by the NC Board of Nursing.Experience:Previous experience in mental health, substance use treatment, or community-based care preferred.Skills:Strong organizational and communication skills.Ability to provide compassionate, client-centered care in a multidisciplinary team setting.Why Join Carolina Outreach?Comprehensive Benefits:Health, dental, and vision insuranceFlexible Spending Account (FSA) and Health Savings Account (HSA) optionsCompany-paid basic life insuranceVoluntary supplemental benefits, including Accident, Critical Illness, and Short-Term Disability coverage401(k) retirement planAdditional Perks:Paid Time Off (PTO)Mileage reimbursement for required travel (using personal vehicle)Opportunities for professional growth and advancementReady to Make a Difference?If you are a licensed nurse passionate about providing holistic, community-based care to individuals with mental health needs, we want to hear from you.
