Come talk to us about joining the Centerstone team!JOB DESCRIPTION:SUMMARY OF POSITIONThe Psychiatric-Mental Health RN (PMH-RN) in Supportive Housing Services is a member of the interdisciplinary team which partners with individuals to achieve independence in their physical and mental health care needs. They provide health promotion, screening, maintenance, and education based on the individual's integrated health care needs. Assessment and monitoring of health care, case coordination, and self-care needs are part of the ongoing PMH-RN nursing process. Administration and oversight of treatment regimens as ordered by the prescribing provider. Support of non-clinical staff related to patient's medical/clinical needs. Management of all patient/client care needs related to medication, point of care lab, and infection control.ESSENTIAL DUTIES & RESPONSIBILITIESInterdisciplinary team memberProfessional communication and collaborationProvide medical/clinical education and support for staffProvide life skills and wellness education and support for patients and familiesParticipate in the implementation of the Individualized Treatment PlanAssist in facilitating linkage to health care servicesAssist in discharge planning, as appropriateAdmission nursing assessments upon move-in to the group home, includingGeneral head-to-toe physical assessmentMedication ReviewAllergy ReviewVerify TB test resultsEnsure any ordered homecare equipment is present and working properly (CPAP, O2, Ambulation Aides)Ensure Primary Care and Psychiatric Care engagementMedications:Medication reconciliation and availability for self-administrationMedication Administration Record (MAR) managementOversight and management of controlled medicationsAdministration of medications on site, as applicableMedication disposalLabs & Waived CLIA Testing:Perform approved Point of Care (POC) testing, as orderedMonitoring and reporting of lab resultsInfection & Exposure Control:Support staff with implementation of Infection & Exposure Control policies and proceduresProvide education and support to staff and patients/patient/clients as needed:Regulatory and compliance documentation and record maintenanceProfessional Requirements:•Meets dress code standards, appearance is neat and clean•Completes annual education requirements•Maintains regulatory requirements•Maintains and ensures patient confidentiality at all times•Reports to work on time and as scheduled; complete work within designated time•Represents the organization in a positive and professional manner in the community•Wears identification while on duty•Completes in-services•Attends staff meetings as assigned•Complies with organizational policies regarding ethical business practices•Communicates the mission, ethics, and goals of facilityKNOWLEDGE, SKILLS & ABILITIES•Effectively communicate via written, verbal, in person and virtual methods•Basic computer knowledge•Strong prioritization/organization skillsQUALIFICATIONSEducation LevelHigh School diploma or equivalent.
