Responsibilities The Health Navigator will: • Support the design, coordination, and implementation of the Refugee Health Promotion (RHP) Program and any other Health Navigation programs. • Organize and deliver educational workshops about various healthcare topics with the goal of increasing health literacy. • In collaboration with JFCS team members, coordinate MediCal or RMA enrollments and initial health appointments for newly arrived refugees and other clients. Coordinate follow-up appointments and referrals. • Provide individual mental health/health navigation, to the degree of the client’s need, to ensure individuals are able to navigate and access complex health care systems. • Transport clients to health appointments or coordinate other transportation assistance. • Conduct health intakes and develop personalized Plan of Care (PoC) and conduct monthly check-ins. • Assist clients with prescription refills and pharmacy navigation. • Collaborate with other JFCS team members and/or community partners to offer various wellness groups for refugees or other clients. • Track health outcomes, collect and analyze data, maintain health related case notes • Outreach, maintain and improve existing community relationships to further community access to health programming. • Research and maintain current knowledge of Orange County and LA County providers, charitable resources and other issues impacting the program Other duties assigned Qualifications Skills/Abilities That are a Must-Haves: • Bachelor's degree in public health, health promotion or social work or equivalent OR at least 3 years of professional experience working as a benefits manager, case manager, client advocate, or other community health position with direct client contact • Familiarity with MediCal and the California health care system • Valid California Driver's license and reliable vehicle with current registration and insurance • Authority to work in the United States • Ability to work with people from diverse cultures in a respectful, culturally sensitive manner • Strong organizational skills with the ability to juggle multiple tasks, set priorities and effectively manage time, and meet deadlines; strong attention to detail. • Ability to contribute both independently and as a team member, to take initiative and follow instructions • Proven written and oral communication skills including the ability to diversify approach when working with diverse communities • Demonstrated ability to build and maintain relationships with clients, customers, community service providers and/or businesses. • Ability to work occasional early morning, weekend, or evening hours • Excellent problem-solving skills, and ability to provide conflict resolution • Computer literate, with proficiency in Microsoft Office Suite and ability to navigate other database software Skills/Abilities We’d like you to Have: • Experience working with immigrants, refugees, asylum seekers or other displaced individuals • Knowledge of nonprofit environment and/or social service resources in Orange or Los Angeles counties • Experience working in refugee health and programs
