EMR: Epic Credentialing Information: 90 days - need to fully credential Travel Preferences: Preferred Lodging: Air Travel – Local; Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation.
