This FQHC in Connecticut is an established organization offering an excellent opportunity for an experienced Family Practice Nurse Practitioner or Physician Assistant who enjoys community health and working as part of the coordinated care team. They would like to speak with APPs with at least 2 years of related experience who will provide primary care services to patients across the lifespan. Board certification as an FNP or PA and Connecticut license or eligibility are expected.
