Description

POSITION SUMMARY

Responsible for providing superior patient care utilizing nursing best practices. Will work in a collaborative and effective manner within an integrated team environment.

ESSENTIAL RESPONSIBILITIES Takes patient vital signs and communicates results to prescribers. Checks AHCCCS/MHS verification or any other eligibility requirements. Takes nursing calls as scheduled. Processes lab results, requests for medication refills/issues and requests by prescriber for transfer. Maintains accurate, thorough, and current documentation of contact with patients and services received/provided. Is proficient in Rems, Altheas, Rapid Result Tests (UDS), PFT, EKG, TMS, AIMS Testing, Med reconciliation & refills (Training protocol). Must be diligent with emails and voice mail transfers to RN level. Works at the direction of clinical teams to remove barriers and assist families to access benefits and community services. Always practices compliance with regard to quality measures. Performs other duties as assigned by Director of Nursing. Qualifications

QUALIFICATIONS

21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positions Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card Must possess a valid Driver License CPI and CPR/First Aid certification required (training provided by Polara Health) Minimum of a High School Diploma or GED required Completion of accredited medical assisting program. Certificate of Medical Assisting or higher Minimum 1 year, prefer 2 years of experience Experience in behavioral/mental health preferred CORE COMPETENCIES
