SUMMARY OF POSITION:

Position is currently funded for 2 years.

The Population Health Clinical Value Associate is integral in supporting Population Health activities. Population Health strives to enhance the patient experience, improve quality, reduce costs for a patients' care and improve care transitions. This individual will need to adjust interventions in a manner that is sensitive to different cultural, religious, economic, and social groups and how it affects the knowledge, delivery, and acceptance of healthcare services. This individual is responsible for supporting primary care objectives through targeted outreach to our patients, data mining, surveillance of health maintenance areas, coordination of care with the broader care team at the clinic as well as Pharmacists, Care Managers, and Social Work. This individual will support monitoring of work queues and proactively create relationships with providers and patients to ensure optimum health and wellness for our patient population. The ideal candidate is someone who takes initiative and can work independently, is outgoing and friendly, and builds relationships with ease. A quick-learner with strong critical thinking skills and who has the ability to be flexible will excel in this role.

ESSENTIAL DUTIES : The percentage of time spent performing essential functions is 95%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

Track and analyze quality metrics and closing care gaps. Supports clinicians in achieving quality incentives through patient outreach (telephonic and electronic outreach). Maintain accurate and timely documentation in the EMR system and provide regular feedback to patients' PCPs. Works to identify different panels of patients, and reviews reports related to chronic disease management and high-risk patients to assist in chronic care management. Act as a navigator for assigned patients and facilitate communication amongst other providers caring for the patient. Develop collaborative partnership with providers caring for the patient through frequent telephonic and/or in person interactions with the provider and office staff. Coordinate patient visits with PCP and specialists as needed. KNOWLEDGE, SKILLS & ABILITIES:

Knowledgeable of healthcare quality programs including Medicare, HEDIS Metrics, and Merit Based Incentive Program (MIPS) preferred. Ability to think creatively and strategically and develop innovative solutions to challenging problems. Possess and maintain intermediate computer skills to include working knowledge of Word, Outlook, Excel, and the ability to learn other software as needed. Experience and knowledge of electronic health records and data analytics preferred. MINIMUM QUALIFICATION(S):

High School Diploma or equivalent required. Must meet current eligibility to sit for the exam with attainment of certification or registration through one of the Penn State Health approved agencies for Medical Assistants within 90-days of employment date or current active status as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) through one of the Penn State Health approved agencies for Medical Assistant certification required.
