We are seeking a dedicated RN Clinical Supervisor to join our Home Health team. You will oversee and guide a team of professionals in providing quality care to individuals in the home setting. This role requires clinical expertise, experience with chart audits, and exceptional time management.

Responsibilities:

• Monitor and evaluate the effectiveness of care plans
• Communicate with collaborating physicians or specialists regarding patient care and changes in condition
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Ensure the accuracy of patient medical records
• Provide advice and emotional support to patients and their family members
• Other duties as assigned

Qualifications:

• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills

• An active, unencumbered RN license required
