Alternative Community Resource Programs Inc is hiring: Psychiatric RN in Altoona

The Alternative Community Resource Program is seeking a Psychiatric Registered Nurse for our Psychiatric Outpatient program and Blended Case Management program.

Responsibilities include:

• Advocates for the health of the consumer by:
  • Referring, and facilitating access to quality physical and behavioral health services intended to meet individualized needs
  • Facilitating access and coordination of behavioral and physical health care services
  • Being responsive to the needs expressed by the consumers themselves;
• Assist and coach clients to advocate for themselves in meetings with health care providers;
• Develop and implement programs around compliance to medication and medical appointments;
• Develop wellness materials for clients on identifying and managing chronic health conditions;
• Develop preventative wellness plans for clients new to the system to preclude the development of chronic health conditions i.e. weight management, nutrition, health screenings, exercise, and smoking cessation;
• Conduct medication management appointments
• Check Client Vitals
• Complete Prior Authorizations
• Monitor clients enrolled in the Wellness Program.

Educational/ Experience Requirements:

• Registered nursing license in the state of Pennsylvania
• At least of one year of nursing experience
• At least one year experience in mental health and/or drug and alcohol abuse

Required Clearances and Licenses:

• PA State Police Clearance
• Act 31 ChildLine Clearance
• FBI Clearance
• Act 31 Certificate
• Valid PA Driver's License with use of reliable transportation
