What youll be doing:

• Implement and monitor individualized patient care plans in the cardiovascular unit.

• Coordinate with all disciplines to ensure seamless patient care.

• Assess discharge needs and facilitate planning to support patient transitions post-care.

Requirements:

• Experience: Minimum of 2 years as a Medical Surgical RN.

• Skills: Proficiency in patient care, discharge planning, and team coordination.
