Job Title: Field Care Management RN
Location: Duluth GA 30097
Coverage Areas: Gwinnett County, Stone Mountain, Covington, Oxford, Monroe, Social Circle, Grayson, Loganville, Snellville
Days Per Week: M-F until shift is complete
Pay: $49.00 - $51.00 per hour – not per visit
Scheduled pay: Weekly
Job Type: Full Time

Care Management Registered Nurse (RN) Responsibilities

• Assess and evaluate the patient’s physical, psychological, and mental needs
• Educate the patient and their family on end-of-life care
• Develop and implement a plan of care that addresses the patient’s needs
• Monitor the patient’s progress and adjust the plan of care as needed
• Maintain patient records and provide written reports on the patient’s condition
• Communicate effectively with other members of the healthcare team
• Participate in team meetings and provide input on patient care

Care Management RN (Registered Nurse) Requirements

• Associates Degree of Nursing Required
• Active RN GA State License
• Valid driver’s license
• Minimum 1 year Home Health experience required
• 1 year Oasis experience required
• 1 year Home Care Home Base experience required
