MMP Staffing Services is seeking a dedicated and compassionate Registered Nurse (RN) Case Manager to join our healthcare team. The RN Case Manager will be responsible for coordinating care for patients, collaborating with healthcare providers, and ensuring continuity of care throughout the healthcare continuum. The ideal candidate will have strong clinical skills, excellent communication abilities, and a commitment to patient advocacy.
Key Responsibilities:
- Care Coordination:
- Assess patients' healthcare needs and develop individualized care plans in collaboration with interdisciplinary teams.
- Coordinate with physicians, specialists, therapists, and other healthcare providers to ensure seamless transitions of care.
- Monitor patients' progress, evaluate outcomes, and adjust care plans as needed.
- Patient Advocacy:
- Advocate for patients' rights, preferences, and needs within the healthcare system.
- Educate patients and their families about treatment options, resources, and self-management strategies.
- Address patient concerns and facilitate access to appropriate services.
- Utilization Review:
- Conduct utilization reviews to optimize healthcare resources and ensure cost-effective care.
- Monitor length of stay, readmissions, and discharge planning to promote efficient utilization of services.
- Collaborate with payers and insurance companies to obtain authorization for services.
- Documentation and Reporting:
- Maintain accurate and thorough documentation of patient assessments, care plans, interventions, and outcomes.
- Prepare and submit reports, including utilization data, quality metrics, and case summaries.
- Interdisciplinary Collaboration:
- Collaborate with interdisciplinary teams, including physicians, nurses, therapists, social workers, and case managers, to coordinate care and support patient goals.
- Participate in multidisciplinary rounds, meetings, and case conferences.
- Patient and Family Education:
- Provide education to patients and families about disease management, medications, treatment plans, and post-discharge care.
- Ensure patients understand their rights, responsibilities, and options for care.
- Quality Improvement:
- Participate in quality improvement initiatives, process enhancements, and performance improvement activities.
- Identify opportunities for improvement in care delivery, patient outcomes, and patient satisfaction.
Qualifications:
- Education:
- Bachelor of Science in Nursing (BSN) from an accredited institution; Master's degree preferred.
- Licensure and Certification:
- Current licensure as a Registered Nurse (RN) in Maryland, Virginia and/or Washington DC
- Certification in Case Management (CCM) or related specialty preferred.
- Experience:
- Minimum of 2 years of clinical nursing experience, preferably in case management or care coordination.
- Experience working with diverse patient populations and complex healthcare needs.
- Skills:
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Proficiency in care coordination, discharge planning, and utilization management.
- Knowledge of healthcare regulations, reimbursement systems, and quality metrics.
Work Environment:
- The position requires working in a clinical and office setting, with occasional travel to patient homes or healthcare facilities.
- Some evening or weekend hours may be required to accommodate patient needs and team meetings.
