Job Description
MMP Staffing is currently seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services in support of a well-known organization. As an LMSW, you will play a key role in addressing the social and emotional needs of individuals and families within the community. This position offers the opportunity to make a meaningful impact on the lives of clients while working collaboratively with a dynamic team of professionals.
Key Responsibilities:
1. Assessment and Case Management: Conduct thorough psychosocial assessments to identify clients' strengths, needs, and risk factors. Develop individualized care plans in collaboration with clients and interdisciplinary team members. Coordinate and monitor the implementation of services and resources to address clients' social, emotional, and practical needs.
2. Counseling and Psychotherapy: Provide evidence-based counseling and psychotherapy to individuals, couples, families, and groups facing a variety of challenges, including but not limited to mental health disorders, trauma, substance abuse, and relationship issues. Offer crisis intervention and support to clients in acute distress.
3. Advocacy and Resource Referral: Advocate for clients' rights and access to services within the organization and the broader community. Connect clients with appropriate resources and support networks, including healthcare providers, social services agencies, housing programs, and financial assistance programs. Provide guidance and assistance with navigating complex systems and accessing entitlements.
4. Community Outreach and Education: Develop and implement community outreach initiatives and educational programs to raise awareness of social issues, promote mental health wellness, and reduce stigma. Facilitate workshops, support groups, and informational sessions on topics such as stress management, coping skills, parenting, and healthy relationships.
5. Collaborative Care Coordination: Collaborate with interdisciplinary team members, including physicians, nurses, therapists, case managers, and educators, to ensure coordinated and integrated care for clients. Participate in case conferences, treatment planning meetings, and multidisciplinary rounds to address clients' holistic needs.
6. Documentation and Compliance: Maintain accurate and up-to-date client records, progress notes, and documentation in accordance with professional standards and regulatory requirements. Ensure compliance with HIPAA regulations and organizational policies related to confidentiality and privacy.
Qualifications:
- Master's degree in Social Work (MSW) from a CSWE-accredited program.
- Current licensure as a Licensed Master Social Worker (LMSW) in the state of DC, MD or VA.
- Minimum of two years of post-graduate social work experience, preferably in a clinical or community-based setting.
- Strong clinical assessment, counseling, and crisis intervention skills.
- Knowledge of social work theories, principles, and interventions.
- Excellent communication, interpersonal, and advocacy skills.
- Ability to work effectively in a multicultural and interdisciplinary team environment.
- Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
