Responsibilities:
Conduct psychosocial assessments and develop treatment plans for clients
Provide individual and group therapy sessions
Collaborate with healthcare professionals and community resources to coordinate care for clients
Advocate for clients and provide support in navigating healthcare and social service systems
Maintain accurate and timely clinical documentation
Participate in case conferences and interdisciplinary team meetings
Stay current with best practices and ethical standards in the field of social work
Qualifications:
Master's degree in Social Work from an accredited institution
Current LMSW or LCSW-C license in the state of practice
Clinical experience working with diverse populations
Strong communication and interpersonal skills
Ability to work effectively in a team and independently
Passion for social justice and advocacy
